Referrals & Admissions

We accept referrals from inpatient facilities, Detoxification units, Community-Based Agencies, EAP (Employee Assistance Programs) and Individuals who fit the Admission Criteria.

All candidates must:

  • Have alcohol and/or chemical dependency as a primary diagnosis.

  • Be at high risk of relapse without continued care in a structured and supportive setting.

  • Be abstinent from chemical and mood altering drugs, with the exception of those prescribed by a physician familiar with the candidates addiction treatment.

  • Appears to be free of serious communicable disease that can be transmitted through ordinary contact.

  • Have recently completed an inpatient chemical dependency treatment or rehabilitation program.

  • Require outpatient chemical dependency services, or educational and/or vocational services in addition to the supportive counseling provided in house.

  • Be capable of self care and participating in all scheduled activities.

  • Not be a danger to themselves or other.

  • Not have Mental Health diagnosis that is beyond our ability to obtain local services for.  Please call (315) 379-0139 with any questions about our limitations.

  • Be at least 18 years of age.

  • Meet ASAM Placement Criteria Second Edition Revised Section 819.3 (d) or current placement criteria as required by New York State Office of Alcohol & Substance Abuse Services.

Step One

We receive referral information that allows us to place the individual on our waiting list.  The information we require can be provided by completing the short referral information form and supporting documentation. We have a first come, first serve policy and do not “bump” anyone, with one exception. If an individual is not ready for admission at the time or within days of having an open bed, we will go to the next person on the list to avoid “empty bed days”. The person that was first on the list continues as first.

We MUST receive and review the following:
Completed Referral/Admission Form (see link above)
Psychological and Social History
Psychiatric or Mental Health Evaluations
Medical and Physical History
Recent Lab reports
Results of a TB test
LOCADTR

Step Two

If it appears that the prospective resident could benefit from our program and that we can provide a bed at discharge (or within a reasonable time after discharge) financial arrangements are verified.  The prospective resident is asked to read the Community Residential Services Handbook.

Step Three

A screening interview by phone that takes thirty minutes to an hour is scheduled.  Our admission decision is made at the end of the interview.  If a candidate's primary counselor is available an admission date is set.

All of our screenings are done by phone to save time and expense for all concerned.  To give the prospective resident information about us and prepare them for the interview we ask that they have read the Community Residential Services Handbook completely before the interview.

We encourage that the candidate to read the Community Residential Services Handbook because we believe that the candidate should be making as an informed choice about admission to our program as possible.  There is no way that all of the material in the handbook can be covered during an interview.  Without reading the handbook the candidate cannot make an informed decision regarding admission.

Cost of Services

We accept the Congregate Care rate which includes individuals receiving Public Assistance, Social Security Disability, Social Security Retirement, Supplemental Security Income, Veterans Administration Disability Pension, Veterans Administration Retirement Pension; or Private Pay. 

North Country Freedom Homes provides community residential services to recovering individuals to promote and maintain abstinence from alcohol, other mood-altering drugs and substances.
