Referrals may be made by individual, family, clergy, physicians, employers, community agencies and courts. It is necessary that legal releases be completed in their entirety before a resident is admitted for inpatient treatment. Requests from agencies for follow-up on residents without releases cannot be honored. With our Criminal Justice System Release legal agents will be informed in writing of the resident's primary counselor, admit date, discharge date and aftercare notices. If the referral is involved with any of the following list of legal agencies a legal release must be filled out.
- Probation/Parole
- Family Court/Child Protective Services (CPS)/Foster Care Worker
- Attorney/Public Defender
- Alternative to Incarceration
- T.A.S.C.
- Legal Aid Society
- Intensive Treatment Alternative Program (ITAP)
- Courts/Judges (Mandated, Pending Warrants or Pending Charges)
- Legal Mandate Letter
- Drug Court
- District Attorney
On each release the name of the person, title, agency, address and phone number is necessary to complete the form. No white-out is permitted. Corrections must have one line through the mistake and initialed.
